Fix Dokan integration settings and UI components.

Summary

Two small fixes are needed in weForms_Dokan_Integration to align with the latest Dokan UI/Settings changes:

• The settings section should use an image URL field (icon_url) instead of a Dashicons slug.
• The allow_vendor_contact_form setting input type has changed from checkbox to switcher in recent Dokan versions.
